Intake Coordinator – Remote

If you’re an LPN who likes fast pace, clean workflows, and talking to patients all day (without the floor chaos), this is a solid lane. You’ll complete intake assessments for newly enrolled chronic care patients and help coordinate the resources that keep them stable.

Schedule

  • Full-time, 100% remote
  • Monday–Friday, no weekends
  • Rotational on-call: about once per year on average
  • Shift options (CST):
    • 8:00 AM–4:30 PM
    • 9:00 AM–5:30 PM
  • Most of your shift is on the phone
  • Remote requirements: high-speed Wi-Fi + HIPAA-compliant home office/workspace

What You’ll Do

  • Accept warm transfers from the Patient Enrollment team for newly enrolled patients
  • Conduct preliminary health assessments and clarify patient needs through open dialogue
  • Identify and coordinate community resources to support care plans
  • Provide patient education and improve health literacy around chronic conditions
  • Support medication management (adherence concerns, refill coordination, potential issues)
  • Help coordinate timely services like Home Health, DME, Home Infusion, and other critical needs
  • Take thorough notes in real-time and keep documentation accurate

What You Need

  • Active Compact/Multi-State LPN (LVN) license
  • At least 3 years of direct, patient-facing experience
  • Strong written/verbal communication and a calm, confident phone presence
  • Comfort working quickly in a fast-paced environment with high call volume
  • Technical aptitude with Microsoft Office Suite
  • Strong attention to detail and strong documentation habits

Pay

  • Starts at $21/hr
  • Ability to earn up to $28/hr based on production
